MZ Medical Billing

Group Practice Medical Billing Services

When a group adds providers, it does not just add claims. It adds enrollments to track, coding styles to align, locations to keep active, and more places for revenue to slip through.

A new physician sees patients before every payer has linked them to the group. A partner bills a new patient visit for someone a colleague saw last year. One location checks eligibility at every visit, while another skips it on busy days. Each problem is small. Across 10, 20, or 50 providers, they add up fast, and nobody has a clear view of where the group is losing money.

That is the billing problem specific to group practices: the more the group grows, the harder it gets to run billing as one operation.

MZ Medical Billing works as the central billing office for your group. We handle the revenue cycle for every physician, NP, and PA under your group NPI and Tax ID, from eligibility and credentialing through coding, claim submission, payment posting, denials, appeals, and A/R follow-up.

You keep your existing EHR, practice management system, payer contracts, and front desk workflow. Our team fits into your current setup across every location, without forcing the group to rebuild its operations around a new billing process.

Where your locations bill differently, we bring them onto one standard. We review how charges move from each provider and site to the claim, find where claims and payments get held up, and give every location the same clean workflow.

Your group stays as it is. Your billing starts working like one operation.

Who It's For

Is Group Practice Billing Right for Your Practice?

Billing gets harder when several providers, locations, specialties, and payer relationships all work under one Tax ID. MZ Medical Billing is built for groups that need one billing operation across all of them, including:

  • Physician groups under one group NPI and Tax ID
  • Single-specialty groups with several physicians, NPs, or PAs
  • Multi-specialty groups with different coding and payer rules
  • Multi-location practices
  • Growing groups adding providers or locations
  • Groups replacing in-house billing
  • Groups switching billing companies
  • Groups formed through mergers or acquisitions
  • Groups with inconsistent billing between sites
  • Groups with growing or aging A/R

Group Practice Billing For All Your Needs

One Billing Office for Every Provider and Location

A group's revenue cycle runs through many hands: front desks at each location, providers with different documentation habits, and payers with different rules. We handle every stage for the whole group, so tasks are never split between locations, local billers, and outside vendors.

Stage 01

Before the Visit

Patient, provider, and payer line up before the visit happens.

  • Eligibility and benefits verification at every location
  • Provider network status by payer
  • Prior authorization
  • Credentialing, reassignment, and group linking
Stage 02

After the Visit

Each encounter becomes a clean claim under the correct group and rendering provider.

  • Charge entry by rendering provider
  • CPT, HCPCS, and ICD-10 coding
  • Same-specialty, global period, and split/shared checks
  • Claim scrubbing and electronic submission
Stage 03

After the Claim

Every claim from every provider is followed through payment.

  • Payment posting by provider and location
  • ERA/EFT reconciliation
  • Contract-level underpayment review
  • Denial management, appeals, and A/R follow-up
Group Level

Across the Group

Leadership gets one view of how the whole group is performing.

  • Reporting by provider, location, specialty, and payer
  • wRVU and collections data for compensation
  • Denial trends by root cause
  • Patient balances and statements for all locations
One billing team for every provider, from the first eligibility check to final payment.
Common Billing Risks

Billing Risks That Make Group Practices Outsource Their Billing

Every practice goes through the same billing steps. What changes in a group is scale. These are the areas where one setup error can repeat across the whole group.

Solo practice One provider affected

A setup error may affect one provider and a limited number of claims.

Group practice Repeats across the group

The same error can repeat across providers, locations, and payers before anyone notices.

01

Billing vs. Rendering Provider

Group claims go out under the group's Type 2 NPI and Tax ID, with each provider's Type 1 NPI as the rendering provider. If the provider isn't linked to the group with that payer, the claim is denied, even when fully credentialed.

02

Medicare Reassignment

Every provider must reassign Medicare benefits to the group through CMS-855R. Without an approved reassignment, Medicare rejects claims billed under the group.

03

New Provider Enrollment Gaps

Commercial credentialing can take months. A new hire who sees patients before enrollment is complete creates claims that can't be paid or must be held.

04

Departing Providers

When a provider leaves, their reassignment and payer links must be ended. Left open, they cause payment errors and compliance problems for the group.

05

Location Enrollment

Each location must be on the group's Medicare enrollment and payer records. A new office that isn't added can cause denials for every provider who works there.

06

Shared Patients

A patient seen by any same-specialty physician in the group within three years is established for all of them. New patient codes billed for them are denied or recouped later.

07

Coding Differences Between Providers

When partners code the same visits at different levels, the group loses revenue on undercoded visits and draws audit attention on overcoded ones. Payer reviews often look at the whole Tax ID.

08

Location-Level Turnover

When a local biller leaves, billing for that site slows or stops, even if the rest of the group is running fine.

09

Contract Underpayments

One contract covers every provider. If payments are never checked against contracted rates, the same underpayment repeats across the whole roster.

Credentialing, reassignment, location enrollment, roster changes, coding consistency, and contract review are all part of the work we handle.

What You Get

What You Get When You Outsource Your Group's Billing to MZ

Many groups run billing through a mix of local staff, a practice manager, and one or two experienced billers who hold most of the knowledge. With MZ, your group gets a full billing department built to work across every provider and location.

A Central Billing Team

Your group is worked by specialists, each handling the part of the cycle they know best.

  • CCAAPC- and AHIMA-certified coders for coding review and provider feedback
  • BSBilling specialists for claim submission and payment posting
  • DSDenial specialists for appeals and underpayments
  • CRCredentialing support for enrollment, reassignment, and roster changes
  • AMAn account manager who runs your group's account
If one person is out, no location's billing stops.

One Point of Contact for the Group

Your account manager knows your providers, locations, payers, and open issues. They bring leadership what needs a decision, bring providers what needs their input (a missing signature, appeal documentation, an authorization detail), and coordinate the rest of the team.

Roster and Enrollment Management

A provider-by-payer enrollment matrix shows who is active, so schedulers book in-network providers before the visit, not after the denial. It tracks:

ProviderPayerLocationEffective date

Your Existing Systems

We work inside the EHR and practice management system you already use, and across systems if locations differ.

Daily Claim Management

Claims from every location are submitted and worked every business day, so problems never age for weeks at one site.

A/R Ownership

Unpaid claims are our job. We work them by payer, age, and value, so high-dollar claims are followed first.

Monthly Group Reporting

Partners and administrators see where the group stands without pulling reports from each office.

  • Charges, collections, and wRVUs by provider
  • Results by location and specialty
  • Denials and their root causes
  • A/R aging by payer
  • Underpayments found and recovered
Switching, Starting Out, or Handing Off

How Getting Started with MZ Works for Groups

The steps are the same no matter where you're coming from. Most of the work falls on our side; your part is mainly granting access and answering questions. See our full onboarding process.

  • In-house billing office
  • Another billing company
  • Merger or acquisition
  • Rapid growth into new locations

Group Billing Review

We learn your group before we change anything.

  • Switching companies: we review A/R, denials, collections, coding by provider, and payer results.
  • From in-house: we also look for unsubmitted claims, unworked denials, and incomplete enrollments.
  • After a merger: we review both setups, enrollments under each Tax ID, and legacy A/R.

Roster and Enrollment Check

We confirm every provider's enrollment, reassignment, and group linking with each payer, and every location's place on the group's enrollment. Gaps are fixed before they cause denials.

Billing Handoff

We tell you what we need and help you find it:

  • Provider roster and payer enrollments
  • EHR and practice management access for each location
  • Clearinghouse information
  • Payer contracts and fee schedules
  • Existing A/R and current workflows

Leaving another billing company? We request your claim and A/R data from them and plan around your end date so no claims go unworked.

System Setup

We connect to the systems your group already uses, map every provider and location, set up ERA and EFT, and load contracted fee schedules for payment checks.

Claims Start Moving

New claims from every location go through coding, scrubbing, submission, posting, and follow-up. Existing A/R is worked at the same time, so older balances don't wait behind new claims.

Ongoing Management

After the transition, your group receives monthly reporting, account support, denial trends, enrollment updates as the roster changes, and A/R status for every provider and location.

Your providers keep working the way they do now. The billing work moves to us.

Why Payer-Specific Billing Matters for Groups

Each payer has its own enrollment process, linking rules, authorization requirements, and bundling edits. In a group, one missed rule repeats across every provider who bills that payer. We track these rules for your whole roster and appeal every denial and underpayment that should have been paid.

Group-Level Rules

Group-Specific Billing Requirements We Manage for You

Group practices follow the same payer rules as everyone else, but several apply differently when providers share a Tax ID, a specialty, and a patient panel. We apply each one to every claim.

Requirement
The Rule
How We Handle It

01New vs. Established Patients

If any physician of the same specialty and subspecialty in the group saw the patient face to face in the past three years, the patient is established for every physician of that specialty.

We check group visit history before assigning new patient codes.

02Global Surgery Across Partners

The global surgical period covers post-op care by other providers of the same specialty in the group. A partner's routine post-op visit is not separately billable.

We apply global periods at the group level.

03Incident-To Billing

NP and PA office follow-ups can be billed under a supervising physician at the full rate when the physician started the plan of care and direct supervision is in place. New patients and new problems are billed under the NP or PA.

We decide this visit by visit.

04Split/Shared Visits

In hospitals and facilities, a visit shared by a physician and an NP or PA is billed by the provider who performed the substantive portion, with modifier FS.

We review notes to pick the right billing provider.

05Same-Day Visits by Multiple Providers

Two same-specialty providers seeing a patient on the same day are usually paid as one visit. Different specialties can bill separately.

We check same-day claims across the group to stop duplicate denials.

06Multi-Location Claims

Each claim must show the correct service location and place of service, and that location must be on the group's enrollment.

We match each claim to the right site.

Modifier Accuracy

Modifiers That Matter Most in Multi-Provider Billing

Group practices use the same CPT and HCPCS codes as any practice. What changes is how they're reported when several providers share patients and procedures. The modifier often tells the payer who did the work and why two similar claims are not duplicates.

Shared Visits and NPP Billing

2 modifiers
FSSplit or shared E/M visit in a facility setting
SANP or PA working with a physician (payer-specific)

Global Surgery Across Same-Specialty Partners

6 modifiers
24Unrelated E/M during the post-op period
25Separate, significant E/M on the same day as a minor procedure
57Decision for major surgery
58Staged or planned procedure during the post-op period
78Unplanned return to the operating room
79Unrelated procedure during the post-op period

Surgeons Working Together

6 modifiers
62Two surgeons, each performing a distinct part of one procedure
66Team surgery with three or more surgeons
80, 81, 82Assistant surgeon, minimum assistance, and assistant when no qualified resident is available
ASNP or PA assisting at surgery

Repeat and Distinct Services

7 modifiers
76Repeat procedure by the same provider
77Repeat procedure by another provider in the group
59Distinct procedural service
XE, XS, XUSeparate encounter, separate structure, unusual non-overlapping service
XPDistinct service by a different practitioner

In-House Diagnostics and Coverage

4 modifiers
26 / TCProfessional and technical components
Q5Reciprocal billing arrangement
Q6Locum tenens physician

Behind every modifier sits the same requirement: the rendering provider on the claim must be enrolled with the payer and linked to the group. Our certified coders check both before any multi-provider claim is submitted.

Group Finance

The Financial and Billing Challenges of Managing a Group Practice

Provider Productivity and Compensation Reporting

Most groups pay physicians at least partly on productivity, using wRVUs, collections, or both. When reports are late or can't be traced back to claims, compensation becomes a point of conflict between partners.

  1. wRVUs by Provider

Work RVUs per provider per month, based on codes actually billed and current Medicare RVU values.

  2. Charges and Collections by Provider

Charges, payments, adjustments, and write-offs tied to the rendering provider for collection-based pay.

  3. NP and PA Attribution

For incident-to and split/shared visits, both the billing provider and the provider who did the work are reported, so APP productivity is credited fairly.

  4. Claim-Level Traceability

Every number traces back to individual claims, so any partner can see how their totals were built.

Group Payer Contract Management

A group holds one contract per payer for every provider. That gives the group more leverage than a solo physician, but only when payments are checked against the contract.

  1. Fee Schedule Loading

Each payer's contracted rates are loaded by code into your billing system.

  2. Underpayment Detection

Every 835 payment is compared to the contracted rate. Short payments, wrong multiple procedure reductions, and incorrect modifier cuts are appealed.

  3. Payer Scorecards

Payment speed, denial rate, underpayment rate, and average payment per visit for each payer.

  4. Renewal Support

Volume and payment data your group can bring to contract renegotiation.

Group MIPS and Value-Based Reporting

Your group's MIPS score affects Medicare payments for every provider under your Tax ID. We manage group reporting so your clinicians get credit for the care they already deliver.

  1. Reporting Path Selection

We model your score as individuals, as a group, as an APM Entity, or through MVP subgroups, and choose the path that scores highest for your TIN.

  2. Quality Code Capture

CPT Category II codes and quality G-codes are added at charge entry, so measures like blood pressure control, A1c, and depression screening count toward your score.

  3. HCC Recapture Tracking

Patients whose chronic conditions haven't been coded this year under the V28 model are flagged, keeping ACO and Medicare Advantage risk scores accurate.

  4. Care Gap Reports

Providers get lists of open care gaps for patients already on the schedule, so gaps close during routine visits.

In-Office Ancillary Billing for Groups

Lab, imaging, physical therapy, DME, and infusion services add strong revenue to a group, but only when they are billed under Stark Law rules.

  1. Correct Billing Entity

Every ancillary claim is billed under the group's billing number or the performing or supervising physician, as the in-office ancillary services exception requires.

  2. Location Verification

Each service is billed from a qualified same-building or centralized-building location listed on your enrollment.

  3. Component Billing

Imaging and diagnostic tests are split into professional (26) and technical (TC) components, with anti-markup rules applied to purchased tests and interpretations.

  4. Supervision Checks

Each diagnostic test is matched to its required supervision level and supervising physician.

Hospital Rounding and Call Coverage Billing

When several partners round on the same patients and share call, hospital charges are easy to lose and easy to bill wrong. Learn more about our hospital and hospitalist billing.

  1. Rounding Charge Capture

Hospital census lists are reconciled against submitted charges daily, so no admission, subsequent visit, or consult goes unbilled.

  2. Same-Day Partner Visits

When two same-specialty partners see a patient on the same day, the work is combined into one correct visit instead of a duplicate claim.

  3. Critical Care Time Across Providers

Critical care time from multiple group providers is added up and billed as 99291 and 99292 under Medicare's shared time rules, with modifier FS when an NP or PA is involved.

  4. Admission and Discharge Accuracy

Initial hospital care, observation, and discharge services (99238/99239) are billed by the right provider on the right date, even when patients change hands at shift change.

FAQs

Group Practice Billing FAQs

What are group practice medical billing services?

Outsourced billing and revenue cycle management for practices with two or more providers billing under a shared group NPI and Tax ID. They cover coding, claims, payment posting, denials, A/R, credentialing, and reporting for every provider and location.

How is group billing different from solo billing?

The steps are the same, but scale changes the risk. Group claims are billed under a Type 2 group NPI and Tax ID with each provider as rendering provider, so enrollment, reassignment, and group linking must be right for everyone. Rules like shared patient status, shared global periods, and same-day visits also apply across the whole group.

Can you bill for all our providers and locations under one account?

Yes. One central team bills every physician, NP, and PA across all locations through a single workflow, with one account manager and one set of reports for the whole group.

How do you handle credentialing for new providers?

Credentialing starts from the day the offer is signed. We handle Medicare reassignment through CMS-855R and link the provider to your Tax ID with every commercial and Medicaid plan, tracking progress in a provider-by-payer enrollment matrix.

What happens when a provider leaves the group?

We end their reassignment and payer links so open enrollments don't cause payment errors or compliance problems, and update the enrollment matrix so scheduling reflects the change.

Can you show which provider is in-network with which payer?

Yes. Our provider-by-payer enrollment matrix shows who is active with which payer, at which location, from which date, so schedulers can book patients with in-network providers before the visit.

Can you fix coding differences between our physicians?

Our certified coders apply one coding standard to every provider. We compare each provider's E/M levels and add-on code use with peers, so undercoded visits capture earned revenue and outliers are corrected before a payer audit.

Do you provide reports for physician compensation?

Yes. We report wRVUs, charges, and collections by provider, credit NP and PA work for incident-to and split/shared visits, and make every number traceable to individual claims.

Can you bill NP and PA services correctly?

Yes. We decide incident-to billing visit by visit and bill split/shared facility visits with modifier FS under the provider who performed the substantive portion.

Do you work with multi-specialty groups?

Yes. Our coders are trained by specialty and work as one team, so each specialty gets focused knowledge while the group keeps a single billing process.

How do you make sure payers pay our contracted rates?

We load each payer's contracted fee schedule by code and compare every 835 payment against it. Short payments, incorrect multiple procedure reductions, and wrong modifier cuts are flagged and appealed.

We just merged with another practice. Can you combine billing?

Yes. We review both billing setups, provider enrollments under each Tax ID, and legacy A/R from the acquired practice, then bring everything into one workflow, including EHR/EMR data migration if needed.

Will switching billing companies interrupt our cash flow?

We plan the handoff around your current company's end date and request your claim and A/R data from them so no claims go unworked. New claims and existing A/R are worked at the same time.

Do you support groups in more than one state?

Yes. We support groups across all 50 states and manage state-specific Medicaid programs, payer requirements, filing rules, and enrollment at the location and provider level.
